The Community Health Needs Assessment and Health Improvement Plan (CHNA&HIP)
initiative is a comprehensive reporting process to assist communities across
Iowa
in communicating their community health needs and planning initiatives.
The overall goal of the CHNA&HIP initiative is to assure the health of
Iowans through a comprehensive and uniform report on leading health priorities
and health improvement plans. Local boards of health were charged with the
responsibility of assuring that comprehensive community health needs assessments
and health improvement plans were completed as described in Iowa Administrative
Code 641-77.
In
February 2005, every county in Iowa
submitted a CHNA&HIP report to the Iowa Department of Public Health. IDPH
has created a centralized, consistent method for local agencies to report the
results of their independent community-based health status assessments and to
document community health improvement plans from 99 Iowa
counties. Each county's report reflects the identified public health
priorities and associated health improvement plans established through their
community health needs assessment process.
To
access the CHNA&HIP reports, complete the following steps:
1.
Go to www.idph.state.ia.us/chnahip
<http://www.idph.state.ia.us/chnahip>.
2. Select the 2005 CHNA&HIP Reports by County.
3. Select the county name from the drop-down list
and click on the "Get Report" button to display the report components
that are available for download. Only counties that appear in the drop-down list have reports that are available
for download at this time. Additional counties will be added as soon as
their reports have been submitted and approved.
4.
Select either a component of a county’s report or select the complete report.
More
information will be made available to counties on how to manage revisions to
their CHNA&HIP report. IDPH appreciates the efforts of all local
boards of health, county public health agencies and the many community partners
that have made a commitment to promoting and protecting the health of all
Iowans.
