SKILLED HOME CARE SERVICES provide support to individuals who are sick, disabled, or convalescing at home. Anyone who is concerned may make a referral for services.

Who is eligible?

Individuals living in their own home or apartment

Individuals discharged from hospitals, nursing homes or group homes

Individuals residing in Calhoun and surrounding counties

Children with multiple medical needs related to acute or chronic conditions

What is the fee?

Home care costs may be covered by one of several sources. Our staff will assist you to identify payer source and process claims. The Calhoun County Department of Health is a Medicare and Medicaid certified agency, approved by Medco, Wellmark Blue Cross-Blue Shield and multi-company health insurance PPO.  Grant funds from the Iowa Department of Public Health and Elderbridge are also available.
